Clinical Info

Ankylosing spondylitis (AS) is a rheumatic inflammatory disease that belongs to the group of spondyloarthropathies. Human leukocyte antigen B27 (HLA-B27) is strongly associated with AS. There has been an increasing appreciation of HLA-B27 positivity in facilitating the diagnosis of an early stage of AS. About 90% of AS patients are HLA-B27 positive, compared to 5-10% of the general population. In addition, an increased prevalence of HLA-B27 has been reported in patients with Reiter's syndrome, anterior uveitis, psoriatic arthritis, and inflammatory bowel disease. As a result, tests of HLA-B27 are a valuable adjunct in the diagnosis of these diseases.
Limitations: This test does not rule out the B*27:06 and 27:09 alleles, which are not associated with spondyloarthropathies. Certain rare alleles present in less than 1 percent of the population will not be detected. Other rare, or uncharacterized alleles may occur which may lead to false positive or false negative results.
