Requisition is completed to include: Name and location of the referring clinician are mandatory Source of specimen: Urine, Bronchial Wash, Pleural effusion, etc Specimen collection date Patient demographics: name, address, date of birth Significant patient history and/or ICD-10 codes Containers are tightly sealed and placed in plastic biohazard bag. Requisition is placed in the pocket on specimen bag. Specimen is submitted to courier.
