Clinical Info

The Cytopathology requisition form must be completed with the following information:
- Name and location of the referring clinician
- Source of specimen, include side, if applicable
- Specimen collection date
- Patient demographics including name, address, gender, and date of birth
- Test(s) ordered
- ICD-10 code (reason for test)
- Significant patient history
- Indicate if special stains, such as GMS or AFB are needed

CytoLyt Solution Vial (Blue top), ThinPrep  PreservCyt® vial (for Anal pap only), 
70% Alcohol Vial for fixed slides, submit air-dried slides in slide holder.

Collection Instructions

All specimens (slides and specimen containers) must be labelled with the patient’s demographics.
- The specimen container must be labeled with the patient's last and first name, date of birth and specimen
source.
- The frosted end of the glass slide also must be labeled with patient’s full last name and first name, and date of birth.
- If the slide is received unlabeled or misidentified, case will be held and the physician will be notified. It is the
physician's responsibility to correct errors in specimen identification.
Entire collected specimen should be submitted in the container vial. It is essential to add fixative (preservative)
to all cytology specimens.
- Exception: for pleural, pericardial and ascites do not submit more than 4 vials. Do not submit vacutainer.
A completed Cytology requisition form, with all the elements listed in Clinical Info, must accompany each specimen
Packaging:
-Tightly seal containers and place them in a plastic biohazard bag. The requisition is placed in the outside
pocket of the specimen bag.
  
Send the specimen to the lab.

Monday through Friday
48 hours analytical time from time of receipt in the lab. Additional time may
be needed if special/immunohistochemical stains/molecular studies are needed.
