ABO-incompatible living donor renal transplantation has become an accepted treatment for end-stage renal disease. Two main factors appear to be important when crossing the ABO barrier, the donor organ A/B antigen expression and the amount of recipient anti-A/B antibody. Antibody titration can be a valid guide for apheresis treatment, which is aimed at eliminating the anti-A/B antibodies in the pre-transplant period and maintaining a low titer in the post-transplant period. Transplant titrations should be tested at room temperature and at IgG phase.
Isoagglutinin production may vary in patients with certain pathologic conditions. Decreased levels of isoagglutinins may be seen in patients with acquired and congenital hypogammaglobinemia and agammaglobulinemia.
