CML is a myeloproliferative disorder characterized by the presence of the Philadelphia (Ph) chromosome. This chromosome is the product of t (9; 22), a reciprocal translocation between the long arms of chromosomes 9 and 22. The corresponding fusion gene, BCR-ABL1, is transcribed into an 8.5 KB mRNA, with 2 junction variants b2a2 (40% of cases) and b3a2 (55% of cases).  The b2a3 and b3a3 transcripts represent less than 5% of cases.  The fusion gene encodes for a chimeric protein, p210, with elevated tyrosine kinase activity.

Methodology

Real-Time Polymerase Chain Reaction (PCR)
CML and ALL are myeloproliferative
disorders characterized by the presence of the BCR::ABL1 translocation
also called Philadelphia (Ph) chromosome. The product of the translocation is
the p210 protein present in CML patients and the p190 protein observed in 20%
of ALL cases. CML and ALL patients undergoing therapy can be monitored with
this test to assess treatment response and detect early relapse. P210 and p190
is quantified by reverse transcription real-time PCR.  CML results are
provided in the international scale, IS.
Health care providers must indicate
the translocation to be tested:  p210 (Major) or p190 (Minor). If not
indicated specimens will be tested for p210 translocation only. Check
p190 (Minor) and/or p210 (Major) on the Hem-Oncology requisition, Cerner
Allscripts or via email to   MolecularPathologyDivision@Northwell.edu
