Collection Instructions	Submit only 1 of the following specimens: Blood Container/Tube: Lavender-top (EDTA) tube(s) Specimen: 2 mL to 5 mL of EDTA peripheral blood Transport Temperature: Ambient Collection Instructions: Deliver specimen to laboratory STAT. If there is a delay in transport, send specimen frozen in plastic vial. Note: 1. Pertinent medical information including patient’s age, suspected diagnosis and a signed “Patient Consent Form” are required on request form for processing. 2. Indicate blood on request form. 3. Label specimen appropriately (blood). Bone Marrow Obtain 2 mL to 3 mL of EDTA bone marrow in a lavender-top (EDTA) tube(s). Invert several times to mix bone marrow. Forward promptly at ambient temperature. If there is a delay in transport, send specimen frozen in plastic vial. Note: 1. Pertinent medical information including patient’s age, suspected diagnosis and a signed “Patient Consent Form” are required on request form for processing. 2. Indicate bone marrow on request form. 3. Label specimen appropriately (bone marrow).
