Collection Instructions

1. Obtain a Para-PakTM Stool Specimen Collection Kit.
2. Using collection spoon provided, obtain stool specimen from areas which appear slimy, bloody, or watery and place into each of 2 containers.
3. Do not fill above line indicated on container.
4. Thoroughly mix specimen.
5. Label container with patient’s name (first and last), date and actual time of collection, and type of specimen.
